Waiver and Release and permission to Participate----- I/We the parent(s) of the above named (hereafter referred to as “said”) child hereby give our permission for the child’s participation in the activity mentioned above. I/We assume all risks and hazards incidental to his/her participation, including transportation to/from activities. I/We do hereby waive, release, absolve, indemnify, and agree to hold harmless Union County Sports , the organizers, sponsors, supervisors, board members and coaches and any other volunteer working on behalf of the Association, including those transporting said child to/from activities from any claim arising out of injury to said child, whether the result of accidental injury, negligence, or any other cause except and to the extent and in the amounts covered by accident and liability insurance. I/We also acknowledge that said child has had a complete physical in the last twelve months by a qualified physician and to the best of our knowledge, said child is physically able to participate in the above mentioned activity. I/We agree to provide satisfactory proof of said child’s age to UCS representative. I/We further acknowledge that we are the legal guardians of the said child and am/are fully authorized to enter into this agreement. I/We also recognize that UCS will employ video and web services during these youth activities and acknowlege that full right and release of the images taken during the camp by UCS for any sales, marketing and promotion of UCS for future camps and youth sports activities.
